Your pattern appears to be overall garter stitch (knit every row) with some decreases on every other row. Now you are to "work" 2 rows, rather than the previous 1 rows. In keeping with the garter st both rows should be knit.

i wonder if you can help me again please.
1st row-right side knit
2nd row- increase in 1st st,k4,increase in next stitch,k1
increase 1 st as before at end of next 3 rows then at each end of 2 following alternate row
increase 1 st as before at each end of 4th row
....
I'm confused at the alternate part, do i knit a row then increase or increase then knit a row and increase.
also in confused to the 4th row it refers to.
